Tennessee Health Dept. Discipline

NASHVILLE, Tennessee (WDEF) – The Tennessee Department of Health took these disciplinary actions in April for our area:

Licensee: Brett Spradlin, D.C., License No 2726, Cleveland
Violation: Failure to maintain continuing education.
Action: Agreed citation; assessed civil penalty in the amount of $600.00

Licensee: Douglas Stithem, D.C., Lic. No 2566, Chattanooga
Violation: Immoral, unethical, unprofessional, or dishonorable conduct.
Action: License is publicly reprimanded; assessed one (1) Type A Civil Penalties in the
amount of six hundred dollars ($600.00), for a total of six hundred dollars
($600.00) to be paid within twelve (12) months from the effective date of the order;
and assessed costs not to exceed five hundred dollars ($500.00) to be paid within
twelve (12) months from the issuance of the Assessment of Costs.

DENTISTRY

Licensee: Nicole Welch Meyer, RDH 3934, Chattanooga
Violation: Failure to maintain continuing education
Action: Agreed citation; assessed civil penalty in the amount of $300.00; proof of
completion of deficient continuing education hours

Licensee: Ricky Patel, DDS 10880, Chattanooga
Violation: Respondent failed to provide proof that during the 2019-2020 continuing
education cycle he completed forty (40) continuing education hours including two
(2) hours in the subject of chemical dependency and had CPR certification.
Action: License reprimanded. The Respondent was ordered to pay $600 in civil penalties,
pay costs, make up deficient continuing education hours, and submit proof of
current CPR certification.

Licensee: Kayle Raines, RDA 22443, Chattanooga
Violation: Failure to maintain continuing education
Action: Agreed citation; assessed civil penalty in the amount of $150.00; proof of
completion of deficient continuing education hours

Licensee: Mary Elizabeth Spencer, RDH 7926, Cleveland
Violation: Failure to maintain continuing education
Action: Agreed citation; assessed civil penalty in the amount of $300.00; proof of
completion of deficient continuing education hours

Licensee: Natasha Wright, RDA 24596, Chattanooga
Violation: Failure to maintain CPR certification
Action: Agreed citation; assessed civil penalty in the amount of $150.00; proof of valid
CPR certification
