Erlanger Awarded $1 million from BlueCross BlueShield of Tennessee
(WDEF) Blue Cross Blue Shield awards Erlanger one million dollars to help promote health care in the community.
Erlanger used the award to create a Care Transition Team.
Its goal is to target high-risk patients, age 65 and older who have no primary care physician and have repeated visits to Erlanger’s Emergency Room within six months.
Now a member of the Care Transition Team follows up with a phone call within one week of an ER visit.
